Please Note!! Although we endeavour to meet all requests for reagents, these resources are finite, hence we reserve the right to use our discretion when dispatching reagents.

Scientists requesting reagents must complete the current version of the CJD Resource Centre requisition form.  Completed forms can be submitted by FAX, though the original signed copies must also be sent via post.  Scientists are requested to complete the Release forms clearly and completely.  Failure to complete all relevant sections may result in the delay of reagents being dispatched. 

If the address to which the invoice should be sent differs from the delivery address, please ensure that this address is clearly stated in your reagent request, together with a contact name. 
Invoices for reagents will normally be issued 2-3 weeks after receipt of the reagents.
